Can a nurse practitioner see patients in a family practice office where the collaborating physician does not see patients and is not on site ever? Can the NP bill under the physician's name for those plans that do not recognize NPs? This is in the state of Florida.

If they can see the patient fall under scope of practice in your state.For incident-to it needs to meet both Incident-to guidelines and fall under scope of practice

the ability to evaluate and treat patients comes with the license granted by the state. And in Florida yes NPs may evaluate and treat patients. However how to bill them is a different matter. If you bill their services using the physician NPI in field 24J then the visit must meet the incident-to parameters. While incident-to is a medicare regulation, if you query carefully you will find that all other carriers will follow Medicare. This means that the provider must be on site in the office suite area while the patient is being seen by the NP, among other things, in a nutshell to qualify as incident -to
* Pt must be established
* diagnosis for the current encounter must have been established by the physician in a previous visit
* there must be an established plan of care that is being adhered to
* the supervising provider must be physically present in the office suite area at the time of the encounter.

For those plans that credential NPs you bill under the NPs NPI. For those plans that do not credential NPs you have to ask them how to bill for NP services. The answer will usually be to bill under the physicians NPI.
