I would like other opinions on number of elements in HPI: Patient is here for solu-medrol infusion. As discussed last night (in ER previous night) per patient. She is supposed to get them daily for the next 4 days. Patient will be coming in at this time the next 3 days following. No problems with infusion except slight excitation a few hours after per patient.

None - there's no problem listed there; just a treatment plan. You can't have any 'history of present illness', if no illness is documented. Hope that helps! ;)
 
Not only are there no HPI elements, based on the intent of the visit there should be no E/M billed at all, just the procedure.
