Can I bill for the NP for subsequent hospital visits when there's no change in patients diagnosis (history) just routine rounds instead of the regular physician- say patient has been in hospital 10 days.

Incident-to is confusing to me. Can someone explain it to me in easier terms?

Incident -to means youare billing under the physician NPI in box 24J when some other qualified person was the one face to face with the patient. The parameters are:
The physician must initial see the patient and establish the diagnosis with a plan of care that will include follow up visits and the plan of care must be documented in the record
The NPP must follow the plan of care with no changes and no new problem
The physician must be present within the office suite area.

You cannot be using incident to billing outside of the physician office setting. If the NPP sees the patient in the facility setting then you use the NPP NPI in box 24j.
