Our nurse sees a DM patient and did a quick Blood sugar check on an a long established patient (under MD orders) in our clinic. She documents vitals, status, and gave advice to our patient. Isn't that a 99211 with a DM dx?
 
All the above should be documented in the chart and the physician will need to counter-sign the nurse's note. But since there was medical advice given along with the vitals and blood sugar check it probably would qualify for the 99211.
 
99211 visit

The doctor's visit with this patient happen before I came here. It is an established patient and a return visit. Vital were documented, light history was discussed, and then advice was given. The doctor will sign it, but it still makes me feel strange charging it. Any more thoughts?
