What exactly do you expect to see in a nursing note documentation wise. also, do you expect there to be a note every single time she sees a patient, even if only for a vaccine? I am trying to write a policy for nursing staff on what and when to document an actual note, seperate from where they enter in injection and medication info. Thanks
