when billing for Nursing home patients and we are billing for the 30/60/90 day follow checks, are these billed using the subsequent codes set of 99307-99309? Or is the 30 day used as initial 99304-99306 and the 60/90 billed as subsequent. A little guidance please!
 
99304-06 are used as initial NH visits. Anything following that would be coded 99307 and on. If a pt is discharged from NH and re-admitted, you can use the initial visit code again.
