If a resident has cancer and is receiving radiation but it is not the reason the resident is staying in the nursing home, do you still code the cancer diagnosis?

My doctor codes cancer diagnosis but she normally sees the patient for cancer related pain, so I always make cancer associated pain (338.3) as my primary diagnosis and cancer code as secondary.
