My physician makes monthly routine visit to the nursing home to see a total of 14 patients. He does not open a visit in the EMR for that visit. We scan in a copy of the hand written dictated note that goes in the chart of the nursing home. Sometimes there is very little on it. It might say "Patient sitting in chair with cheerful disposition. Patient is smiling and states no complaints. Blood pressure 130/90. All other VS stable. Continue present management."
How can one get an E&M for that? I have talked with others at the hospital and some are using a pre printed form with boxes for their docs to check off as they do them. Is that enough documentation to bill for an E&M?
