I have a patient coming in for a nursing service injection. Its her own medication. Would it be appropriate to code a 99211 and do I need a modifier for this?

I agree...the injection code only and the fact that the patient brought in her own meds should be documented in the chart.
 
If you have a provider in the office (direct supervision), then you would bill 96372. The physician does not need to be in the room with the nurse during the injection, he/she must be present in the office suite. You only bill the 99211 if a physician is not available. Some carriers require you to add the name of the medication and the NDC number when the patient supplies the medication.
