Wiki OB check and other problem

If a pt who is pregnant visits office for cold and routine ob/gyn check, do we code the cold as if it complicates the pregnancy--what are the rules regarding the documentation--does the provider have to state "does not complicate pregnancy" or "does complicate pregnancy."

What resource did you find the reference in? I have this with several flu patients who are also pregnant this month.
