You are on the right track when you state you need to code for the reason the patient came to L&D. L&D is also an outpt triage setting--the nurse evaluated the patient and contacted the physician for appropriate direction. I would not use V22.1.
You can use 789.00 or 724.2 or even 625.9 with 648.93 or 644.13 or 644.03 depending on the weeks. I would definitely use the UTI IF the lab results confirm--646.63
