If nurse intake/assessment shows patient weight and BMI which indicates obesity. Patient is 14 weeks pregnancy. Physician does not document and problems withy pregnancy. Should I use ICD 10 diagnosis of Obesity complicating pregnancy or do I use encounter for normal pregnancy. Thanks. :confused:
 
if the provider does not document obesity in the chart note then the coder is not allowed to code it. It is the providers responsibility to document the diagnosis of obesity. a qualified medical person in the office is allowed to document the BMI. if there is no documentation of the obesity by the provider, the coder is not the one qualified to interpret the BMI.
 
Besides, someone can be obese and have an uncomplicated pregnancy. I would use normal pregnancy unless the doctor states that there are complications.
