Hi there,
Does anybody have a set of guidelines or a cheat sheet for OBGYN diagnosis coding??? I'm truly confused with the screening codes, contraceptive management, pregnancy tests, consultations for STD....sigh
I'd like to learn when a routine exam includes the extra codes. Sample scenario below:
Reason for Visit: pt visit for family planning annual exam
Assessment:
Status of Existing Problems:
Assessed CONTRACEPTIVE MANAGEMENT as unchanged
Assessed GYN EXAM NORMAL (W/ OR W/O PAP) as unchanged
Assessed BREAST - CLINICAL BREAST EXAM as unchanged
Assessed STD COUNSELING as unchanged
Assessed DELAYED MENSES as new
Assessed CIGARETTE SMOKER as unchanged
Assessed SEXUAL ACTIVITY, HIGH RISK as unchanged
New Problem:
DELAYED MENSES (ICD-626.8)
Codes captured: 626.8,V25.09,V65.45,V69.2,V72.31,V72.41.
