does anyone know why caresource has started denying codes

for single gestation births? we can't figure this out!
 
If I'm not mistaken, Caresource is Medicaid. Often times, insurance will deny when the patient isn't covered, but the denial reason is not accurate. Did you verify coverage? That's where I start when a denial doesn't make sense.
