I am looking for clarification on documentation requirements for Observation status in ED. If a patient is admitted into Observation in the late evening and discharged on the early morning the next calendar date we can bill admit observation code range (99218-99220) and 99217 for discharge on the next calendar date. Is there a separate E/M encounter needed for the discharge on the next calendar date or would the one record if the encounter suffice? Any guidance would be greatly appreciated.
