Does anyone have any physician guidelines for this service? I am coding for the facility - my problem is that our ED docs are placing patients in observation almost as soon as they walk in the door, and then not sharing this information with other staff (nursing, registration). This leaves them billing for observation, and me reviewing loads of charts to see if they SHOULD be observation.

I have guidelines for the facility, stating there should first be the evaluation, including lab, etc. prior to placing in OBSV, and that the observation should last longer than an average ED visit (4-6 hours). However, I don't know how successful I'll be using this info to convince them that the facility regulations apply to them. CPT doesn't list a minimum time frame...any help out there???
