I would be reluctant to use 26541 because it specifies "with tendon or fascial graft". Have you thought about using 26540-22 for increased services, since this describes more than just the suture fixation?

This is NOT a reconstruction. It is a simple repair. There is no indication for a 22 modifier either.
26540, period.
