can someone explain the difference between obtaining a graft and harvesting a graft. I'm studying for the cpc exam and at times it says that obtaining the graft is included in procedure however harvesting of the graft can be coded additionally.

Same incision vs different incision

Usually you can't code the harvesting of a graft unless it requires a separate incision. Even if you have a separate incision if the basic code INCLUDES harvesting or obtaining the graft then you can't code it separately.

Look at the guidelines under "Grafts (or Implants)" on page 80 of the 2009 CPT Professional edition.
