20551 (trigger point inj) with a 64405 (occipital nerve block inj) - CCI edits state 20551 is bundled with the 64405, but may be billed with a modifier when appropriate - if the 20551 is injected just below the skull, at the highest point of the cervical vertabrae is that considered outside the area of the 64405?
