On the UB-04 claim form, who is typically responsible for attaching occurrence, condition, and value codes? Billers or Coders?

Billers do the occurrence codes on our side if they are not automatically populated

Where I've worked, this is usually not a routine part of the coder's work, and a coder would only assign one of these if the software indicated that it was required due to a particular code assignment or if the billing department or another area routed the claim to coding specifically asking for this. Coders generally aren't trained on the details of claim form completion so it's for the most part outside of their scope.
