I am looking for someone who bill for professional services for a Group. I want to make sure that I have all the correct numbers on the 1500 form. My understanding is as follows:
Box 25 - Group Tax ID
Box 32 - Place of service with Facility NPI
Box 33- Group name address and Group NPI
Box J - Individual Physician NPI
Any other tips or hints you can give me about group billing on the 1500 would be greatly appreciated. I bill for an individual Dr now and I thinks it's pretty much the same except for the above...
