Does anyone use a formula or particular method when setting up or revising your office
fees? Are you simply increasing as necessary to stay ahead of your highest payer?

fee schedule

use a percentage of medicare. fairly standard

I research the allowable on Medicare website and then multiple those fees by a percentage.
Remember you must charge all third party payers Medicare, Blue's, etc the same fee.
CA Workers Comp has its own allowable fee schedule I follow.
