Wiki Office testing -report when done or when the doctor reviews?

Hello, I recently found a claim that the doctor added the testing codes to his encounter that occurred 5 days later. Typically, we have billed the testing separate and on the date of service the test was provided. Is it acceptable for the doctor to add the codes in the EHR for the same day he saw the patient? The test were not billed on the date performed.
 
You'll need to be more specific when you say 'testing'. There are hundreds of tests.....lab, rad, diagnostics, psychiatric, and hundreds of studies in the medicine section of CPT.
Are you talking about his billing of a professional read? That's billed on the date that he does the read. For office/point of care labs, they would be billed on the date of the specimen collection or performance of the technical component. If it's anything else, you'll need to clarify.

And from a charge capture perspective it's generally best to post the charge on the date of service, but you can post a late charge (within timely filing) as long as the DOS is correct.
