Wiki Office visit or Hospital Admit?

If the doctor sees a patient in the office and admits them to the hospital that day, do I code the E/M for hospital admit or just the office visit?

I have always been taught to code the hospital admit, however, an article is stating to code the office visit.

A. Initial Hospital Care From Emergency Room
Contractors pay for an initial hospital care service or an initial inpatient consultation if a physician sees his/her patient in the emergency room and decides to admit the person to the hospital. They do not pay for both E/M services. Also, they do not pay for an emergency department visit by the same physician on the same date of service. When the patient is admitted to the hospital via another site of service (e.g., hospital emergency department, physician’s office, nursing facility), all services provided by the physician in conjunction with that admission are considered part of the initial hospital care when performed on the same date as the admission.

I agree with Rebecca...as long as the provider actually saw the patient in the hospital after the admission...

What if the doctor sees the child (or infant) in the office and decides to admit him after a significant work-up, but the residents do the work that night in the hospital? Does our office bill for the office E/M or the admission?
