just questioning if modifier 25 should be attached to the office visit when a visual field is done or a modifier 51. please help

The visual field examination 92081-92083 would not be valid with the use of modifier 51. If you are inquiring on whether or not the use of modifier 25 or 51 would be appropriate on an office visits (99201-99205, 99212-99215) than only a modifier 25 would apply.
Please remember that for modifier 25 to be justified that the evaluation and management would need to be a significant, separately identifiable E/M service; a service above and beyond the other service provided. So in this case the history, examination, and MDM would need to be above and beyond that which is normally inclusive in the 992081-92083.
