Can anyone tell me where in the notes are OMT notes supposed to be placed?

I think it would depend on the EMR you're using - ours just creates a total separate 'note' on the date that it's filled out. Otherwise, we just get them to fill it out by hand (& uploaded) and the provider can just write the score or anything else into wherever they think it's relevant in the SOAP note.
