POS (22) hospital based clinic, when a patient is seen for a yearly Oncology exam, history of breast cancer, estrogen receptor positive, current use of arimidex, patient also has anemia. Would the diagnosis code order be Z08, Z853,Z7910, Z79811, D649?

Since the patient is still receiving a treatment for the breast Cancer (arimadex) you would code as active cancer. ie C50.???; Z17.0; Z79.811; and depending on why or what type of Anemia that code.
