What CPT code would I use for an oophorectomy performed with a c-section. During the c-section procedure the provider found an undiagnosed ovarian tumor and a decision was made to perform an oophorectomy. 58611 is for removal or ligation of fallopian tubes during c-section. Would 58940 be appropriate? Possible with a modifier 52 for reduced services?

You would code 58940-51 for this. There is no need to report a reduced services modifier as this is a secondary procedure and will be discounted by the payer in any case. The code is bilateral or unilateral so no reduction is needed.
