Procedure: Excise neoplasm, right ear (2.5cm), with retroauricular fasciocutaneous flap and anterior adjacent tissue transfer flap with frozen section control of the margins.

OP Procedure: Attention was turned to the ear where excision was carried out. The closure necessitated a retro auricular fasciocutaneous flap closing the posterior aspect up to the helix and then anteriorly adjacent tissue L-flap was fashioned for closure.

An adjacent tissue transfer includes the removal of the skin lesion. You can code 14301 IF the area of the defect for the ATT (adjacent tissue transfer) is between 30.1 sq cm and 60.0 sq cm. That's a fairly large ATT for the ear. Read the Op report more closely or have the surgeon amend the Op report with the dimension of the ATT closure. You may then consider also 14060 or 14061. As far as the fasciocutaneous flap, I would also code the 15732. That code does not have dimensions.
