When a patient presents for a refill of meds and has new drug induced constipation, would this be considered a separate diagnosis under "Number of Diagnoses or Management Options"? Would I give 1 point for the stable chronic pain and 3 points for the new drug induced constipation OR would I give 2 points since the chronic pain may be stable but the med is causing a side effect (one established not at goal)? I consider it Moderate Risk as a chronic illness (chronic pain) with side effect of treatment (constipation).
