New coder here. I keep getting denials from Optima on 31625 with dx R59.0 stating diagnosis doesn't match the CPT when dx. Any suggestions?
 
I am not seeing an issue with this when I code it out. It may be payor specific. I'd give them a call to see what they're medical necessities are for this Px.
