To code 92002 and 92012 what is the criteria?(# of history elements/exam/MDM) for Optometrist.

The specs for those codes are in the Eye Section of the CPT book. That applies to optometrist as well. Eye codes do not have the same criteria as the E/M codes (history, exam bullets, MDM per se).
