ORIF revision-removal and replacement of hardware

Patient comes in 4 days post ORIF of the talus because a screw was backing out. Surgeon removed the existing screw and replaced it with a shorter one. Would this best be coded with 28445-78? Or 20680 for the screw removal and 27899 for placement of a new screw?
 
Try googling hardware exchange CPT, and if you have CPT Assistant or another resource, how to code hardware exchange and the code 20680. Also try searching the term in the forums here.
