Wiki Ortho doc utilizing athletic trainers

I was wondering if any orhto coders have had their physicians work with athletic trainers? How do you bill for their services? I have found CPT 97005 and 97006 for the AT eval and re-eval...I know that the trainers will also be working on the home exercises and crutch/gait training with the patients. Would I bill these out under the ortho physician?
Any info would be greatly appreciated
