Outpatient Blood Transfusion at Hospital

Hello,

I work for a Hematology/Oncology Practice and one of our physicians orders blood transfusions to take place at a nearby hospital. While the patient is having his/her transfusion, the physician stops by during his rounds and fills out a brief problem focused H&P on a printed hospital form. The physician is trying to charge an Initial Observation Care code of 99218 for this. I am sure that is not correct. So, the hospital bills for the transfusion but can we bill anything for the physician H&P? Would that be visit be considered part of the transfusion service?
