I coded 250.40 and 585.9 is this wrong?
 
Either the disease for which the panel is being run (i.e. hyperlipidemia for a lipid panel) or the signs and symptoms as the reason for the test.

Are you coding for a lab? The ordering provider should provide you with the diagnosis.
