The provider did not document that the loop recorder was extracted in his procedure report, however, it was documented in the procedure log - would you report it?

No. The procedure log is a billing tool. The procedure must be documented in the patient chart. This is an area of clinical risk, so I'd ask the provider to go back and update his/her note.
