I have never billed a UB04 form and I have been trying to submit claims to Medicare but they keep being returned for different reasons. Can anyone give me suggestions on what needs to be where...the last claim was returned because of occurrence code. I billed it with 29 returned needed to have 35 to be returned again needing 29. I called they told me to put both codes in which is different than what I was told the time before. I asked if there was any where I could get info on this and was told no! HELP Please! Thank you in advance!
