Hello everyone,

I’m looking for some guidance on documentation best practices for outpatient physical therapy clinics.

In our state, PTAs are permitted to provide treatment under general supervision, meaning the supervising PT does not need to be physically present in the facility but must be available by phone or other HIPAA-compliant communication methods.

From a compliance and documentation standpoint, would it be considered best practice to include a field in the PTA daily treatment note template to capture the type of supervision present for that specific encounter (e.g., direct/on-site supervision vs. general/off-site but available)?

For example, some possible template options could include:

  • Direct supervision (PT present in clinic)
  • General supervision (PT offsite but available by phone or secure communication)
  • Supervising PT name
I’m curious how other outpatient clinics are handling this:

  • Do you require PTAs to document the supervision type for every visit?
  • Is this something you’ve added primarily for audit/compliance protection?
  • Have you encountered any payer expectations around documenting supervision level in the daily note?
