I am taking a test for outpatient surgery done in a hospital setting. I would be coding for the physician who has performed the surgery. I have reviewed my ICD9 coding book. It states when a patient presents for outpatient surgery, code the reason for the surgery as the first-listed diagnosis (reason for the encounter), even if the surgery is not performed due to a contraindication.

I was always taught to code the confirmed diagnosis? Any thoughts on this...

thats interesting because the rules for ASC coding are different in that we code the findings 1st (depending on the carrier that is)
