How should I code for an outpatient visit(Hem Onc) for an established patient seen by the MD, but 911 was called and patient was transported to the ER? IV fluids was started in the clinic for a fever before being transported to the ER. The patient was admitted into the hospital by the ER under Hem Onc service.

If I understand your post correctly, the patient was admitted by another physician - BUT a physician in the same practice and same specialty as the one who saw the patient in the clinic ...

If I'm correct ... then you only code the initial hospital visit, but you may use ALL the documentation (clinic visit as well as admission notes) to arrive at the level of service. Since you must have a face-to-face encounter with the patient in the setting (i.e. hospital inpatient) in order to bill, the claim will go out under the name/NPI of the admitting physician.
