I've been told when billing a 99213, 99406 and 96372 on same claim modifier 25 needs to be on the 99213 and 99406. I've never heard this, has anyone else?

I also heard about it but I only use modifier 25 for the 99213. Never had any problems.

We only put the 25 modifier on 99213. No problems; been billing this way for years.
