What would be the correct code for "Ovarian failure" with no symptoms if the patient had hysterectomy ? Is it E28.39 (Other primary ovarian failure) or E89.40(Asymptomatic post procedural ovarian failure) ? Appreciated any insight!!

"Ovarian failure" alone codes to E28.39. If the documentation indicates that it was a result of the hysterectomy, then E78.40 would be appropriate, but I would not choose that code solely on the patient's history of a hysterectomy unless the provider has documented the linkage between the condition and the procedure.
