Hypothyroidism stable - Will refill levothyroxine and will return in August for lab work.
Overweight BMI 29 - Will start on phentermine and recheck in a month.

Question is: for overweight is that considered acute uncomplicated problem for complexity? Patient has tried many different diets, exercise, OTC medications, supplements etc. to help with weight loss to no avail so has reached out to a doctor for help. Provider feels it should be a chronic problem not stable/not at goal.

Q2. How would a diagnosis of obesity or overweight count in the number and complexity of problems?
A2. Base your choice of the level on how you are addressing this issue. Documentation must support the AMA definition of a problem addressed. This could be a self-limited or minor problem. This could be a stable chronic condition if the patient is at your goal. This could be 1 or more chronic illnesses with severe exacerbation, progression, or side effect of treatment. Notation in the patient's medical record without additional assessment or care does not qualify as being addressed by the practitioner.
