Wiki PA vs APRN with a new vs established edit

I'm getting a new vs established patient edit. My new patient PA visit is hitting off an APRN visit. Should this be changed to established if the payer is Medicare? I think it should be am being told it shouldn't - the edit is for PA vs PA or APRN vs APRN.
