Packaging Ancillary Charges during an Out patient Hospital stay

If a patent is having a scheduled surgery and staying a few days, do you package ancillary charges only billed on the same dos of the procedure? or all the ancillary charges for the length of entire stay? Is a PT Eval considered an ancillary charge because it is a Therapeutic service?
