Hi i have just started a new job and I just need a few questions answered.
I work at an ASC facility and I would like to know if when you bill for the facility and professional fees do they both go on the same claim form or do they need to be done professional on the CMS 1500 and facility on the UB 04 I know that Medicare only takes 1500 forms any help would be greatly appreciated this is for the year 2017 by the way
