Good afternoon, my doctor has performed a right T11, T12, L1, L2 dorsomedial branch radiofrequency thermocoagulation and I am trying to figure out how to correctly code this. Do I code this as 64633 for T11/T12, 64434 for T12/L1, 64635 for L1/L2?

Thanks,
Liz

Hello namdeoshinde and jadhavganesh345,

I am a bit confused about the codes above, in the 2021 CPT coding book it indicates that 64633 should be used for T12/L1. So how can you use the codes above? I'm very new to coding so I would love some help since my doctor also performed to same procedure as elizabeth.adams' doctor.

Thanks so much for any help and explanation you could provide.

Sincerely,
Erin Albert
