Can anyone help me with the LCD for both cpt codes 64451 & 64625. I have called Medicare and they showed me where to get the LCD but does not give you what you need to get the medical necessity for both cpt codes. Thank you

i got both LCD but don't give you the medical necessity for them. And the max quantity.
