Wiki Pain vs Injury primary dx code

Good day! I have gotten myself confused. If a pt is having a knee xray for knee injury, knee pain - the report is negative. Do you code the pain as primary and the injury as secondary dx? Or what is the proper way to apply the dx codes? Thanks!
